현재 아래처럼 셀렉트박스로 이름과 전화번호중 선택해서 검색할 수 있도록 되어 있는데..

이름 필드와 전화번호 필드를 나누고 두개 모두 일치해야 검색이 될 수 있도록 하려면 어떻게 해야 하나요?
